IAP 115 running 4.2.1.0 randomly blocks websites with OpenDNS

this is odd, it has happened twice to me now where I visit a website and get denied access by OpenDNS when connected to my IAP-115 using 4.2.1.0.

 

OpenDNS is NOT configured on the IAP. Anyone see this before?

I disconnect from the SSID, re-connect, and the problem goes away.

Do you have content filtering enabled at all?  Please PM me your configuration.

no content filtering anywhere.
config was PM'ed.
That evening I upgraded to 4.2.1.1.

If I recall correctly the first time this happened was on 4.2.0.1.

misconfiguration on the latest attempt with 215 and 4.1.3
