I have IAP 205 and CPPM 6.5.4
For guest SSID access i am using sponsor approval. i have configured SMTP server details in messaging setup and successfully getting test email from CPPM.
But i am not getting email on sponsor email id from guest client.

Has this ever worked? Should you set it up from scratch, or did something make it stop working?

This is new setup.
Issue has been resolved by TAC.
Found that the key | value wrongly updated on the email form field.
Updated the proper format and working properly.
Customer getting the sponsor email with hostname, update the sponsor approval template with the IP address.
Also disabled the sponsor login before approval.
