Have you checked to see if "Private Wi-Fi Address" is enabled in the SSID settings on the iOS device? (Settings > Wi-Fi > SSID blue i button > Private Wi-Fi Address)
If enabled it will generate a virtual MAC address to use. If it is not enabled, it will use the actual device wireless MAC. That can also be found under general settings (Settings > General > About > Wi-Fi Address)

MAC Authentication is getting denied state.
