I have switch HP5406R Zl 2 with module connected of 

1. HP 24p Gig-T (J9986A) 

2. HP 12p Gig-T 12 SFP (J 9989A)

The above modules 1 and 2 are connected and shows falulty with orange light in A and B

I tried to update teh firmware KB_16_01_0007.swi

Its shows an error

Both J9986A and J9989A are v3 zl2 Modules.

Both J9986A and J9989A require specifically an HPE 5400R zl2 Switch running Software Version at least KB.15.17.xxxx (first KB.15.17 was 0003 published May 2015) or - better - an updated one of the same branch (as example KB.15.17.0013) or an updated one of a newer branch (like KB.15.18 or KB.16.01).

Is (or was) your HPE 5400R zl2 Switch running KB.15.17 (or newer) Software Version?

Did you follow software upgrade via TFTP/XMODEM instructions (see attached screenshot for XMODEM)?

now the modules are working fine
