Regarding point 1, in practice you don't want to use 40MHz on 2.4GHz band as there isn't enough channels. This is accurately described here in a visual manner https://support.metageek.com/hc/en-us/articles/204490510-40-MHz-Channels
Yes, it is supported on the AP but you should almost never use it!
Regarding point 2, the datasheet lists the lowest & highest MCS index for each combination. These are usually enough to help you do the coverage plan and understand your data rate distribution.
The lowest MCS index will show you how far can the client connect (client will have the lowest data rate as its signal will be low and the SNR will be low and thus you will be using a less performing modulation scheme).The AP will be transmitting at the highest possible transmit power depending on your regulatory requirements.
The higher MCS index will show you the best possible connection that the client can get. This will require the client to be very close to the AP and have a high SNR.The AP doesn't need to transmit at high power in this case.
As such, you now know your closest and farthest point and the other rates can be achieved in between. Needless to say, the client device needs to be able to support this MCS combination.
