Is there a firewall between access points and the controller?

How long has the problem been happening?  How long has the system been installed?  You should type "show log system all" to see if there are any messages that would indicate system instability.

I had this issue when the switch between the controllers and the APs was dropping packets. In my case, this was due to jumbo frames being mishandled.  I suspect that is why cjoseph asked about firewalls.  Probably something along the way is dropping packets.  That is where I would look first.  Check the switches for errors related to dropped or filtered packets.
