Have you checked the output of 'show stacking detail' and verified that all stack port links are displayed as Active? (If you choose to post that output here, make sure it is sanitized by removing serial numbers and MAC addresses first.)
Other things to check as part of the troubleshooting process are the output of the following commands:

Look for any abnormal indicators, such as a power supply present but not powered, a fan not working properly, or an OverTemp value of YES.
If that doesn't help, check the event log with filters for higher severity events ('show logging' with either '-e' or '-m' flags).
If none of these options helps identify the cause, log a TAC case to engage additional resources for troubleshooting the issue.
