You can have multiple Intune Extensions running, so you don't need to uninstall the old one first. Just be aware that you can have only 1 extension using the same extension IP address (think about the HTTP Authentication Source if you use that), and replicating the same data through multiple paths may be a recipe for disaster so I would not recommend to run multiple versions of the extension on the same Intune Instance.
