Sure enough I just swapped the Cisco ESX switch with a Netgear GS724TP and now all clients are connecting.  There must be something on the Cisco switches that causes broadast messages like DHCP to choke.

 

Any ideas?

I also tried an HP 2520G POE switch and I have good results.

This may have something to do with it.  The cisco switch port limiting the amount of DHCP from an IAP mac address.

well it is not limiting the ammount of DHCP, it limits the number of MAC adresses allowed on a port (port security) to prevent people from connecting a switch or such. for a port on which you connect an AP you should never configure port security. certainly not with a low number like 3.
