Error (Telnet/SSH Error: (pattern match read eof) in prompt after telnet password: User Access Verification )"

This message shows that AMP is unable to log in to an AP via telnet or SSH. Likely causes:

1. AMP is using the wrong username, password, or enable password.
2. The user AMP is logging in as doesn't have sufficient credentials. 

In either case, the solution is to edit the credentials that AMP is using, which you can do on either the AP -> Manage page or by clicking Modify Devices from any list of APs.
