PSU issue with HP 5400

Problem:

Diagnostics:

Troubleshooting already performed by local team:
1. Re-seated power supply
2. Replaced the existing power supply

First Occurred Date : Feb 1,2019,Replacement case 5335987720

Detailed Problem Analysis

Getting perm failure for the PSU.
We have replaced both the PSU twice, but same result.
In order to clear this fault i have followed the below plan of action
1. Remove the power supplies.
2. Power down for at least 30-40 seconds, remove the power cable from the power supply. (Important to time this for 30-40 seconds)
3. And then connect both of the power supplies back to the chassis.

Troubleshooting already performed by local team:
1. Re-seated power supply
2. Replaced the existing power supply

Upgraded the firmware as well to the latest, same result.
After upgraded req cu to remove PSU and connect it back, same result.

Faulty PSUs are working fine in another switch and there are false alarms on alarmed switch
While changing to PoE Mode on switch and all the LAN ports are indicating as orange color continuously even that A2 & b1 LAN ports also indicating a same color.

Frequency of issue: Frequent

Cu have multiple switch at the same site, same PSU ,no issue with other chassis.

Solution

Elevete the case to ERT for further troubleshooting if the steps in the known bug do not resolve the issue
